What the National Influenza Immunization Program (NIIP) did not and could not survive was finding cases of Guillain-Barré syndrome (GBS) among persons receiving swine flu immunizations. As of 1976, >50 “antecedent events” had been identified in temporal relationship to GBS, events that were considered as possible factors in its cause. The list included viral infections, injections, and “being struck by lightning.” Whether or not any of the antecedents had a causal relationship to GBS was, and remains, unclear. When cases of GBS were identified among recipients of the swine flu vaccines, they were, of course, well covered by the press. Because GBS cases are always present in the population, the necessary public health questions concerning the cases among vaccine recipients were “Is the number of cases of GBS among vaccine recipients higher than would be expected? And if so, are the increased cases the result of increased surveillance or a true increase?” Leading epidemiologists debated these points, but the consensus, based on the intensified surveillance for GBS (and other conditions) in recipients of the vaccines, was that the number of cases of GBS appeared to be an excess.

Little more than a century ago, life was a minefield of illness and injury. Women died giving birth, children succumbed to contagious diseases, and a compound fracture—a break in the skin as well as the bone—could prove fatal. You were lucky if you survived to age 40. Now we are living twice as long. We can prevent and cure diseases, and repair and replace many body parts.
How was this radical makeover achieved? The history of medicine is usually described as the slow accretion of knowledge over hundreds of years, punctuated in the late 20th century by the sudden convergence of engineering and biology. The only problem with that narrative is that it misses the two factors most responsible for the development of clinically effective medicine: an epochal invention and a scientific rebellion.
Ironically, that invention was nearly overlooked. While it would enable not only modern medicine but all of the life sciences, it languished in the shadow of a related invention.

Specially designed encasements are available that are bite proof, escape proof and entry proof. Encasing mattresses and box springs with bed bug-proof encasements will trap existing bed bugs inside where they will starve and die. In addition, encasements prevent additional bed bugs from getting inside mattresses and box springs. Also, the smooth exterior of the encasement makes bed bug inspection much easier. In the event that infested beds are discarded, encasements can be used to protect replacement beds from becoming infested by bed bugs still present in other parts of the dwelling.

Can the mass professionalization of journalism be applied to medicine or health? Can access to a broadband connection outfit a citizen to think and act like a physician?
There are pieces of what physicians do that can be replicated, and other pieces that can’t. The technical things that doctors do can’t be replaced. Removing an appendix or replacing a heart valve, for example. Tough to pull off on CureTogether.
But what about the thinking? After all, patients have access to the same information, references, and literature as physicians.
Unfettered access to information can create an illusion. It can give us a false sense of control. In his provocative book, You Are Not a Gadget, Jaron Lanier tells us that “information is alienated experience.” And experience is the only process that can dealienate information. Or in the case of the infirm, it’s clinical judgment and experience that makes that myriad of medical literature applicable to an individual’s case.

The scientific concept of illness refers to a bodily lesion, that is, to a material — structural or functional — abnormality of the body, as a machine. This is the classic, Victorian, pathological definition of disease and it is still the definition of disease used by pathologists and physicians as scientific healers.
The brain is an organ — like the bones, liver, kidney, and so on — and of course can be diseased. That’s the domain of neurology. Since a mind is not a bodily organ, it cannot be diseased, except in a metaphorical sense — in the sense in which we also say that a joke is sick or the economy is sick. Those are metaphorical ways of saying that some behavior or condition is bad, disapproved, causing unhappiness, etc. In other words, talking about “sick minds” is analogous to talking about “sick jokes” or “sick economies.” In the case of mental illness, we are dealing with a metaphorical way of expressing the view that the speaker thinks there is something wrong about the behavior of the person to whom he attributes the “illness.”

Autism is the most common condition in a group of developmental disorders known as the autism spectrum disorders (ASDs). Autistic children have difficulties with social interaction and verbal and nonverbal communication, and exhibit repetitive behaviors or narrow, obsessive interests. Autism varies widely in its severity, with symptoms ranging from mild to severe. In some cases the disorder may go unrecognized. Scientists aren’t certain what causes autism, but it’s likely that both genetics and environment play a role.

There is no single known cause of autism. Contributing causes may include:

  • Genetics: There is convincing evidence that autism is a heritable disorder. The identity and number of genes involved remain unknown.
  • Environmental: Epidemiologic studies indicate that environmental factors such as toxic exposures, teratogens, perinatal insults, and prenatal infections such as rubella and cytomegalovirus account for few cases.
  • Toxic exposure: Studies done to date fail to find evidence that immunizations with the measles-mumps-rubella vaccine are responsible for the surge in autism. There has been considerable controversy about a possible toxic or heavy metal (especially mercury) component to autism.

In order to study the cause of ALS outside of the motoneuron and the nervous system in ALS, we have launched a new study on genetic environmental epidemiology. We believe that interactions between genetic predisposition and environmental/behavioral factors and exposures may lead to oxidative stress in patients who develop ALS. In collaboration with Drs. Ottman and Factor Litvak, we have already developed the interview tools for genetic, behavioral and environmental factors. Now, we are conducting a small center-based study in preparation for a future larger population-based study. For genetic factors, we also have been working with Dr. Kottmann, who is investigating motor neuron specific gene expression, and we are also a part of the ALS DNA repository.

Researchers from the London School of Economics have found that people with high I.Q.s are more likely to be night owls, whereas folks with lower I.Q.s are more likely to wake up early and function their best during the day. Other studies have found a link between “eveningness” and getting good grades in school.
